(1) Vendor needs to provide taxi voucher program administration services to the government authority located in Glendale, AZ.– This Taxi Voucher Program for Dial–A–Ride and provide a more personal, individualized service for dialysis patients and residents with other ongoing, repetitive medical needs.– Employ qualified staff with sufficient experience and knowledge to maintain or improve the current level of professional service and effectively manage the Pro–gram.– Provide all necessary personnel, office space, equipment, supplies, administrative support, and materials required for successful program operation– Respond to transportation requests from eligible participants in a timely and efficient manner.– Ensure vouchers include:• Customer name• Origin and destination• Issue and expiration dates (valid for three (3) months)• Required waiver language• Clear and legible format– CUSTOMER SERVICE AND OUTREACH– Respond to customer inquiries and concerns in a timely manner– Provide program participants with a current list of participating transportation providers(2) A pre–offer conference will be held on September 16, 2026.(3) All question must be submitted no later than September 21, 2026.
