(1) Vendor needs to provide rural health transformation program service to the government authority located in Oregon. – Partner with organizations that can implement innovative, culturally responsive, and community–driven approaches to improve health outcomes, reduce barriers to care, and connect individuals to health and social support.– Awarded Proposers may provide services that support rural residents through outreach, engagement, health education, care coordination, system navigation, and referral services. – Awarded Proposers may implement activities that improve access to healthcare and social supports in rural communities and align with Rural Health Transformation Program (RHTP) goals.o Chronic disease prevention and self–management supporto Behavioral health outreach and prevention activitieso Connection to food, housing, transportation, and other social supportso Development of partnerships that improve access to services for underserved populationso Culturally specific and community–driven approaches to addressing health disparitieso Activities that increase the number of rural residents receiving new or expanded health and social health services(2) All the questions must be submitted no later than September 10, 2026.
