(1) Vendor needs to provide complex care coordination program service to the government authority located in Rochester, NY. – The Complex Care Coordination Program is intended to ensure appropriate and efficient placement in skilled nursing or assisted living facilities, or connection to care in otherwise appropriate care settings, of patients who require an alternative level of care less than the acute care provided in in–patient hospital settings or who are otherwise difficult to place based on their medical, behavioral health, or social–emotional needs.• Complex Care Cohort Grants and Support• Support for Assisted Living Facilities and Assisted Living Programs• Development and Expansion of Preventive and Community–Based Care Services• Performance Evaluation and Data Analysis• Long–Term Financing and Sustainability• Compliance and Quality Assurance(2) All questions must be submitted no later than November 13, 2026.(3) The contract period will be two years.
