CASE-1785 - USA (Kingston, New York) - Enhanced Case Management Services - Deadline August 21,2026

(1) Vendor needs to provide Enhanced Case Management Services to the government authority located in Kingston, NY. – The Firm shall provide part–time enhanced case management services which increase the safety of Office for the Aging (OFA) clients in their homes by connecting them with supports and services in the community based on their health/wellness, transportation, and socialization needs.– The Enhanced Case Management (ECM) will provide informal initial and ongoing assessments for the purposes of monitoring the mental, emotional, and physical health of seniors, including grief and loss; to assess cognitive status and changes in order to recommend appropriate intervention or referral; and to encourage activities and socialization opportunities as appropriate.– The ECM will support those seniors who are dealing with physical changes, such as vision, hearing and mobility loss, and help them access resources and interventions that will maintain and/or improve quality of life and ability to remain safely in the home.– The ECM will connect involved family and other caregivers of persons with Alzheimer’s disease, dementia and other age–related mental and physical conditions to appropriate support.– The ECM will work with clients in their home, the hospital, and/or nursing home, as needed.(2) All the questions must be submitted no later than August 21, 2026.
