(1) Vendor needs to provide conducting, tabulating and reporting surveys service to the government authority located in Waterbury, VT. – Provides a wide range of employment–related services for people with disabilities.– Conduct, tabulate and report on two separate surveys as follows:• Task 1 – Consumer Satisfaction Survey (CSS): Conduct, tabulate and report on one survey to determine current consumer satisfaction with services.• Task 2 – Monthly Consumer Surveys for Staff Performance Review (SPR): Conduct, tabulate and report on regular, ongoing consumer surveys to determine satisfaction with services from individual counselors as a part of staff evaluation process.– Provide alternate surveying formats for respondents with disabilities as needed.(2) All the questions must be submitted no later than June 29, 2026.
