The Illinois Department of Innovation and Technology (DoIT, Agency or State) is executing an agency specific contract with a vendor to purchase high speed Internet Egress services. This contract provides a purchasing mechanism for DoIT to purchase high speed Egress services and the ability for DoIT to purchase additional bandwidth in the future, as needed, if needed. This contract is in conjunction with a Universal Service Fund Schools and Libraries Program E-rate Form 470 pursuant to the DoIT Consortium, and Vendor is required to have a valid Service Provider Identification Number (SPIN) from the Universal Service Funds Schools and Libraries Program. As part of this contract, Vendors are required to complete and adhere to the E-Rate Supplemental Terms and Conditions included as Attachment A.
