(1) Vendor needs to provide medical examinations service to the government authority located in Richmond, VA. – Provide required physical examinations and medical tests to selected employees in each of the multiple locations. – Provide medical examinations to employees as directed by the agency. – Provide professional medical services.– Schedule and see new hire employees for initial examination within one week of new hire notification.– Schedule and see employees as required by agency within two weeks of employee contacts. – The contractor of authorized representatives shall meet periodically with the agency, when requested, to discuss all services and/or concerns/complaints.– Provide consultation to the agency’s division of human resources concerning medical restrictions, accommodation, guidance on miscellaneous workplace medical issues and medical related information.• Provide health–screening packets to agency for each covered employee to complete prior to the appointment, packets to include the following:– List of approved service providers with phone numbers, addresses, and hours of operations.– Medical authorization form– Employee information and release of information form– VOSH Medical program – employee history form– Audiogram history/report form– Respirator medical evaluation questionnaire– Tuberculosis screening form– Consent for hepatitis immunization– Hepatitis B vaccine information sheet– Medical care assessment questionnaire(2) The contract period will be for three years.
