Description
Organization Name is accepting bids in response to this Bid Call for Project Title Project ID The objective of this Call for Bids is to contract with a service provider that will provide the best overall value to Organization Name . While price is a significant factor, other criteria will also form the basis of our award decision, as more fully described in the Bid Evaluation and Contractor Selection section of this Request for Proposal below. The project/program may include, but is not limited to: • Replacing identified domestic watermain/waterlines. • Replacing/adding isolation and control valves. • Reconnection of main lines to existing distribution system. Throughout this project there will be a need to maintain quality control and management of cost and expenses. The selected provider will be expected to ensure that all applicable federal, state and local laws and regulations are followed. This project is funded through the ARPA distribution received by Lewis County and the City of Centralia. This work is being performed on an active water system and scheduled events will need accommodation throughout the construction schedule. In addition, this system supplies domestic water to the Twin Cities Senior Center. Bid Submittal and Closing Date Bids shall be submitted through the OpenGov Portal at https://procurement.opengov.com/portal/lewiscountywa . Late submissions will not be accepted. Contractors submitting bids not in compliance with Section 4; Submission Guidelines and Requirements, will be considered non- responsive. Inquiries and Addenda Questions must be received prior to the questions deadline identified in this Bid Call shall be submitted through the OpenGov.com website at https://procurement.opengov.com/portal/lewiscountywa Questions and answers will be posted to the project listed on OpenGov. Addenda to this solicitation, if needed, will be posted to the project listed on OpenGov. The County, at its sole judgment, may require clarification of information submitted in any Proposal. Mandatory Requirements The following submission guidelines and requirements apply to this Request for Proposal: 1. Only qualified individuals or firms with public water system installation and maintenance experience should submit proposals in response to this Call for Bids. 2. Bidders must list at least four (4) projects that are substantially similar to this project as part of their response, including references for each. 3. Bids must include a proposed project schedule that provides an overview of the proposed approach as well as an estimated total duration, not withstanding any unforeseen circumstances or schedule conflicts. 4. Bids must be signed by a representative that is authorized to commit bidder's company. 5. Statement of Conflicts of Interest (if any) the service provider or key employees may have regarding these services, and a plan for mitigating the conflict(s). Note that the County may in its sole discretion determine whether or not a conflict disqualifies a firm, and/or whether or not a conflict mitigation plan is acceptable. 6. System for Award Management. Bidders should have a current registration in the System for Award Management ( https://www.sam.gov/SAM/) . Company and its Principals may not be debarred or suspended nor otherwise on the Excluded Parties List System (EPLS) in the System for Award Management (SAM). Include verification that the service provider as well as its principals are not listed (are not debarred) through the System for Award Management ( www.SAM.gov ). Enclose a printout of the search results that includes the record date. This clearance information should be included in the bid submission packet. The clearance in the Service Provider’s proposal will be re-verified prior to award. Federal awarding agencies may relax the timing of the requirement for active SAM registration at time of allocation in order to expeditiously issue funding. At the time of award, the requirements of 2 CFR § 200.206, Federal awarding agency review of risk posed by recipients, continue to apply. 11. Required Contract Provisions. Applicable provisions (enclosed) must be included in all contracts executed as a result of this project including subcontractors, consultants, and service providers. 12. Proposals must remain valid for a period of 60 days. 13. Submissions must meet the Pass/Fail Criteria listed.
