(1) Vendor needs to provide professional education, promotion activities, and evaluation support service. – The Contractor must support communication and outreach activities to promote chronic disease–related programs, including preparation of social media content, newsletters, educational materials, and data briefs as directed.– Provide support across program areas by coordinating partner engagement, assisting with evidence–based intervention activities, and supporting evaluation, training, administrative, and communication functions as directed.– The contractor must support access to national training resources, evidence–based programs, community workshops, and innovative practices relevant to chronic disease prevention and management. – Improving workforce capacity to address health–related social needs.(2) All the questions must be submitted no later than October 02, 2026. (3) The contract period will be for one year.
