CASE-1783 - USA (Martinez, California) - Spanish Speaking Adults and Partnership Intensive Case Management Services - Deadline August 28,2026

(1) Vendor needs to provide spanish speaking adults and partnership intensive case management services to the government authority located in Martinez, CA. – Provide Full–Service Partnership (FSP) Intensive Case Management (ICM) services for Medi–Cal eligible, Spanish‑speaking adults across county.– Targeted Case Management (TCM): A service that assists a beneficiary in accessing needed medical, educational, social, prevocational, vocational, rehabilitative, or other community services.– Service activities may include, communication, coordination and referral; monitoring service delivery to ensure beneficiary access to services and the service delivery system; monitoring the beneficiary’s progress, placement services, and plan development.– Peer Support Services: Culturally competent individual and group services that promote recovery, resiliency, engagement, socialization, self–sufficiency, self–advocacy, development of natural supports, and identification of strengths.– Ensure translation and interpretation services are available as needed.– Provide referral and linkages to care and support.– Mental Health Services: Individual or group therapies and interventions that are designed to provide a reduction of mental disability and restoration, improvement or maintenance of functioning consistent with the goals of learning, development, independent living, and enhanced self–sufficiency.(2) All the questions must be submitted no later than August 12, 2026.
