(1) Vendor needs to provide school–based case management and youth intervention service to the government authority located in Santa Ana, CA. • Deliver on–site case management, relationship–based mentoring, and conflict mediation services at designated school sites.• Maintain a dedicated, on–site case manager or team assigned to each participating school.• Develop an individualized service plan for each student in collaboration with school counselors, administrators, and, where applicable, probation officers or juvenile court liaisons• Coordinate referrals to community resources, including mental health services, substance use treatment, housing assistance, workforce development, and legal aid, as needed.• Maintain confidentiality and comply with FERPA, applicable juvenile records confidentiality law, and District data–sharing agreements.• Conduct intake assessments for each referred student, including review of attendance history, discipline history, court/probation status (where legally accessible and authorized), and family circumstances.
