Vendor needs to provide functional assessments services to the government authority located in Chattanooga, TN.• Agency seeking to perform functional assessments and make eligibility recommendations for those seeking eligibility and paratransit services.• Paratransit eligibility is directly related to the functional ability of individuals with disabilities to use fixed–route transit services.• Eligibility is not based solely on a diagnosis or type of disability, the type of mobility aid used, age, income, ability to drive, or access to private transportation.• Securing suitable facility(s) for physical assessments or provision of suitable alternative method forconducting assessments.• Provide necessary equipment and software for daily admin tasks (i.e. phone, fax, printer, etc.)• Scheduling assessments• Coordinating travel with Care–A–Van• Securing partnership with interpreter and language access specialists, if required• Establishing comprehensive customer service, including phone and electronic communicationcapabilities
