Vendor needs to provide community health navigational services to the government authority located in Albuquerque, NM.– Deliver high–impact navigation services that reduce barriers to health and social stability for priority populations.– Improving self–reported health, supporting stable connections to health care homes, strengthening health and social service networks, and driving systems–level improvement through collaboration and advocacy.– Responsible for enrolling eligible participants and aligning services with appropriate Pathways to support meaningful outcomes for participants.– Includes identifying recurring barriers, participating in cross–organization collaboration, and contributing to shared efforts to improve access, coordination, and effectiveness of services.– Program will include a post–award transition and collaboration period to support alignment, onboarding, and shared learning across community partners.
