(1) Vendor needs to provide homeless assistance and housing support programs service to the government authority located in Modesto, CA. – Provide Homeless Assistance Program (HAP) and Housing Support Program (HSP) services.– Provide case management services for homeless participants.– Consultant shall complete the Homeless Management Information System (HMIS) data entry for participants from entry date through exit date of the program period.– Conduct landlord outreach and develop a list of landlords willing to rent to participants.– Provide education on how to create a household budget and repair credit.– Provide resources and referrals for non–housing services.(2) Pre–bid meeting will be held on November 18, 2026. (3) All the questions must be submitted no later than November 25, 2026.
