(1) Vendor needs to provide employee health and wellness centre services to the government authority located in New Jersey.– To improve employee health, stabilize benefit plan costs and promote a culture of wellness.– Primary care is to be performed and managed by a personal physician at the centre that shall collaborate with other health professionals and shall utilize consultation or referrals as appropriate.– This care may include chronic, preventive and acute care in both inpatient and outpatient settings. – The physician must be specifically trained to provide comprehensive primary care services through residency or fellowship training in acute and chronic care settings.
