(1) Vendor needs to provide wound specialist physicians service to the government authority located in Paterson, NJ . – Wound specialist physicians to provide timely on–site services in accordance with federal and state regulations for post–acute and long–term care residents. • Clinical responsibilities:– Provide wound care rounds with designated facility staff, on a schedule approved by executive director/designee, as ordered by attending physician.– Perform assessments and treatments as medically necessary pursuant to attending physician order. – Provide assessment and management of wounds, including mechanical, chemical and or surgical debridement as medically necessary.– Provide treatment recommendations consistent with facility formulary.– Discuss wound management recommendations with family, caregivers, attending physicians.– Document assessment and progress of wound conditions in electronic medical record system utilized by facility. – Provide wound care education twice/year for nursing staff.– Provide dermatological services and complete skin biopsies as needed.• Provide quarterly performance improvement reports that must include for each month in the quarter:– Wound/Pressure injury stage.– Healed Wounds– New Wounds: Hospital or Facility acquired.– Rates of healing(2) The contract period will be for two years.
