INSU-6086 - USA (Denton, Texas) - Life Insurance, Short-term Disability and Long-term Disability Service - Deadline July 1,2026

(1) Vendor needs to provide life insurance, short–term disability and long–term disability service to the government authority located in Denton, TX.• Basic Life/AD&D and LTD coverages begin on the date of hire. Supplemental and Dependent Life/AD&D and STD begin on the date of enrollment• The City also offers benefits to eligible dependents (legal spouses, declared common–law spouses, unmarried children up to 26 years of age, children of any age if mentally or physically incapacitated and dependent on the employee• To provide a benefit structure matching the current Life/AD&D benefits. Currently, the City of Denton pays a benefit for Class 1 “All Full–time Active Executives” of 2x salary not to exceed $500,000, and Class 2 “All Full–time and Part–time Active Permanent Employees” of 1x Salary not to exceed $300,000, but will provide at least a minimum of $15,000.• To provide a benefit structure matching the current Supplemental Life/AD&D benefits available in $10,000 increments up to 5x annual earnings to a maximum of $200,000. This is in addition to the basic employer–paid benefits• To provide a benefit structure matching the current Voluntary STD program at 60% of earnings up to $1,200 per week with a 14–day elimination period and an 11–week maximum payout(2) The contract period will be for three years.(3) All the questions must be submitted no later than June 22, 2026
