(1) Vendor needs to provide pre–employment screening, physical exam services and occupational health services to the government authority located in Portland, OR. – Provide pre–employment screenings, physical exams, and other required medical examinations to police department applicants and complete and sign the applicable forms.– Physicians, physician assistants, or nurse practitioners may administer optional annual occupational health services, including physical examinations, to current police department employees.– New Hire Pre–Employment Screening and Physical Exams:• Medical surveillance exam.• Physical exam and medical history review.• Respiratory exam, including spirometry and respiratory questionnaire.• Vision test, including acuity, peripheral vision, depth perception, and color vision.– Annual Occupational Health Services:• Routine annual physicals.• Additional optional services.(2) All the questions must be submitted no later than September 14, 2026.
