EXTRA-77829 - USA (Modesto, California) - Child and Family Team Meeting Services - Deadline December 15,2026

(1) Vendor needs to provide child and family team meeting services to the government authority located in Modesto, CA.– Provide scheduling and facilitation of child and family team meeting services for referred child welfare services and probation department families, youth, and non–minor dependent.– Consultant shall contact the family, youth, and non–minor dependent and child and family team member participants to communicate the time, date, and location of the child and family team meeting.– Consultant shall provide documentation of the child and family team meeting, including the service plan, to the primary agency: community services agency (CSA) or probation, including the family, youth and non–minor dependents within four (4) business days of the child and family team meeting.– Consultant shall schedule one (1) child and family team meeting per youth. the meeting shall be scheduled for a minimum of one (1) hour.– Consultant shall provide a monthly schedule of child and family team to the respective agency.– Identification of the needs and strengths of the youth, and/or non–minor dependent and their family.– Development of shared plan(s) with the family, support plan implementation, modifying plans, as needed and transition family to informal support.– Establishing and monitoring case and treatment plans for both the youth, and non–minor dependent as well as the parent or guardian.(2) All the questions must be submitted no later than November 3, 2026.(3) The contract period will be for one year. (4) Pre–bid meeting will be held on October 27, 2026.
