Vendor needs to provide enterprise referral care coordination center service to the government authority located in Washington, DC. – The RC3 serves as the central hub for care coordination for beneficiaries enrolled in DC as well as the private sector, which includes the Managed Care Support Contractor(s) (MCSC(s)) for referral processing and appointing management.– For all enrolled beneficiaries, the RC3 will actively manage patient transitions of care, securely exchange clinical information between referring and receiving providers, and assist high–risk patients in navigating complex and specialty care pathways.– RC3 care coordination will be customer–centric and include functions such as maintaining high standards for first call resolution, actively identifying beneficiaries who may and should receive direct care, proactively identifying patients requiring multi–disciplinary care, facilitating the synchronization of appointments and services to minimize patient travel, reducing administrative burdens, and ensuring continuity of care and quality or services throughout the referral lifecycle.
