(1) Vendor needs to provide case manager services.– Schedule and participate in case conferences/reviews with the participants, program coordinator and program director regarding service care planning and agreement negotiations.– Provide program discharge planning to dis–enrolled participants to ensure that the participants needs were met.– Act as an advocate for participants with outsourced service providers and others, as needed.– Conducts weekly home visits to participants residence.– Develop, prepare, facilitate and present training and educational programs and workshops.(2) This is not RFP; this is job posting.
