(1) Vendor needs to provide consumer and healthcare reporting modernized application service • Mobile–friendly, desktop–compatible web application that increases usability, improves completeness and quality of submitted information, and reduces user burden.• Enable authorized program personnel to make interface and content updates with limited developer support, using current, state–of–the–art UI/UX design principles• Design and develop mobile–friendly, desktop–compatible web application:• Design and develop a responsive web application optimized for mobile devices and modern desktop browsers, enabling users to access and complete reporting from any device with internet access, including form validation, error handling, and user–friendly data entry to reduce reporting burden.• Implement a pop–up customer satisfaction survey presented upon submission to measure satisfaction with the submission process.• Implement medical record upload and document management: integrate an upload tool for medical record information as part of the form update, while maintaining the current upload tool for documents that become available post–submission.• Free–text box allowing submitters to provide additional information as free text.
