(1) Vendor needs to provide medical examinations services to the government authority located in Richmond, VA.– Provide medical examinations to employees as directed by the agency.– Schedule and see new hire employees for initial examination within one week of new hire notification.– Schedule and see employees as required by within two weeks of employee contacts.– Contractor of authorized representatives shall meet periodically with the agency, when requested, to discuss all services and concerns or complaints.– Personal and family histories.– Work history:– Current exposures.– Physical examinations.– Special and laboratory examinations:– Chest X–ray (14 x 17 P&A) upon hiring and once every five years up to age 45 and every year after age 45.– Pulmonary function test. Audiometric test.– Electrocardiogram.– Urinalysis.– Lead testing.(2) The contract period will be for three years.
