The City of New Smyrna Beach (herein after, “City”) is seeking proposals from interested and qualified Insurance Brokers/Agents licensed in the State of Florida to provide innovative benefit plan design and effective health care choices for approximately 315 full time City employees. Services shall include: Group Health Insurance Group Dental Insurance Group Vision Insurance Group Long and Short-Term Disability Insurance Group Life Insurance and Accidental Death & Dismemberment COBRA FSA and HSA If an award is made it shall be made to the best overall proposer(s) whose proposal is most advantageous to the City, taking into consideration the evaluation factors set forth in this document. The successful proposer will hereinafter be referred to as the "Consultant".
