(1) Vendor needs to provide home visiting services to the government authority located in Hawaii. – Provide Home Visiting Services, a program promoting positive parent–child relationships and supporting families with children ages zero (0) to five (5) years old.– Provide Home Visiting Services according to one of the following two evidence–based home visiting models: (1) Parents as Teachers (PAT) or (2) Healthy Families America (HFA) Child Welfare Adaptation.– Activities listed below:1. Initial Contact2. Assessment, linkage, and coordination3. Individualized Family Plan (IFP) Development4. Implementation of the Individualized Family Plan5. Transition Planning and Case Closure6.
