(1) Vendor needs to provide sober living facility development services to the government authority located in Jefferson, WI.– Human Services will serve as the primary treatment provider for all individuals residing in the sober living home.– Sober living providers must target the funding towards people who have struggled with opiate addiction.– Residents will be referred to human services for assessment, treatment planning, and ongoing behavioral health services.– Plan should detail strategies for securing ongoing financial support, building community partnerships, ensuring staff retention, and implementing program evaluation and quality improvement measures.(2) All questions must be submitted no later than September 15,2026.
