(1) Vendor needs to provide medicaid coverage services.– Secure medicaid nursing home coverage, or recertification of nursing home coverage, to the selected respondent who will provide assistance to residents and families. – Review and assess eligibility criteria for nursing home medicaid coverage.– Provide guidance and support to residents and families in completing Medicaid applications accurately and in a timely manner.– Collect and organize all necessary documents required for the application process.– Liaise with relevant government agencies and facilitate the timely submission of completed medicaid applications, ensuring adherence to the three–month retroactive eligibility period for coverage.– Monitor and track the recertification timelines for residents.– Assist families in completing the recertification forms accurately and in a timely manner.– Collect and organize all necessary documentation for recertification.– Follow up on any pending recertification requests or additional information required.– Maintain accurate and up–to–date records of resident Medicaid applications, approvals, denials, and recertifications.(2) All questions must be submitted no later than October 16, 2026.(3) The contract period will be for one year.
