Description
As reflected in state legislation such as AB 133 and initiatives such as California Advancing and Innovating Medi-Cal ( CalAIM ) and the CalAIM Population Health Management Service, data sharing is crucial to improving care coordination, service provision, and program planning. The Data Exchange Framework ( DxF ) and Trusted Exchange Framework and Common Agreement (TEFCA) of the California Department of Health and Human Services ( CalHHS ) have expanded opportunities for health data exchange. The Social Health Information Exchange (SHIE) for Sacramento County , named “S acramento Health Connect” (SHC ), will provide health and social care data exchange infrastructure for County residents , specifically Medi-Cal members and j ustice - i nvolved population s , to improve care coordination and program planning. Many of the benefits offered by the SHC align closely with the programmatic priorities that the Sacramento County Board of Supervisors has previously identified, including improving service delivery and coordination for people experiencing homeless ness in the County , strengthening behavioral health linkages , and improving ca r e management and warm handoffs for those involved in the justice system. This project will help ensure the County's compliance with the Mays Consent Decree and the continued development, implementation, and monitoring of the Sequential Intercept Model (SIM). For the purpose of this document , SHIE and SHC will be used interchangeably to refer to the same infrastructure . As a strategic outcome, the County envisions the SHIE facilitating the secure and permitted exchange of health and social service information between providers and systems of care, enhancing the ability of healthcare providers and care coordinators to diagnose, refer, prescribe, and/or support individuals in need. Using SHIE data, program planning and client outcomes can be improved. Through data integration and interconnected service systems that are more responsive to social determinants of health, the SHIE will enhance countywide efforts to provide " W hole P erson C are." The County of Sacramento welcomes all qualified vendors or consortium of vendors who will meet the requirements to submit an RFP response to participate in this SHIE (aka SHC) and make the County’s vision successful.
