(1) Vendor needs to provide data management solution.– Design, deploy, operate, maintain, and support a Data Management system.– The system and services shall accurately and effectively track, facilitate, and coordinate beneficiary, participant, care coordinator.– Management and tracking of Money Follows the Person (MFP) transitions;– Case Management System that meets data tracking and reporting requirements.– The messaging system must provide the option for callers to leave a number for a returned call.– Maintain active client lists within the system;• Develop, provide, and maintain an electronic system for tracking and assignment of assessments based on data elements.(2) All the questions must be submitted no later than September 24, 2026.
