Description

See attached Request for Proposal (RFP) 36C24426R0047 for Lebanon VA Medical Center urology physician services, and RFP Attachments D.1-D.4. Lebanon VAMC requires a contractor to provide key personnel to perform onsite urology physician services for eligible beneficiaries of the Department of Veterans Affairs. Contractor shall furnish services at the Lebanon VAMC located at 1700 South Lincoln Avenue, Lebanon, PA 17042 and four associated Lebanon VAMC Community Care Clinics: York VA Clinic, 2251 Eastern Blvd., York, PA; Mechanicsburg VA Clinic, 5070 Ritter Rd., Mechanicsburg, PA; Berks VA Clinic 2762 Century Blvd. Wyomissing, PA; and Lancaster VA Clinic 212 Willow Valley Lakes Dr. Willow Street, PA. The Government intends to award one fixed price indefinite-delivery, indefinite-quantity, (FP-IDIQ) contract, to include an ordering period of five years, to one offeror who can successfully place a competitive proposal to satisfy RFP requirements. This solicitation will be set aside 100 percent for Total Small Business competition. All Contractors must be registered in System for Award Management at www.sam.gov to be eligible for award and complete their online Representations and Certifications at www.sam.gov. The applicable NAICS code is 621111 with a size standard of $16 million in average annual receipts. Award will be made to the offeror whose proposal is responsive and conforming to the RFP and determined to be most advantageous to the Government. The evaluation will be conducted in accordance with FAR Parts 12 and 15 source selection procedures. Proposals will be evaluated on Candidate Qualifications and Experience, Management Plan, Past Performance, Socioeconomic Status, and Price.
