Description

Purpose: The purpose of the Notice is to announce that Network Contracting Office (NCO) 21, intends to solicit proposals from vendors to provide Community Program Support for the Center for Pacific Islander Veterans Health (CPIVH). This project is authorized by the Veterans Affairs Pacific Islands Health Care System (VA PIHCS). Vendor to provide these services in 4 geographic regions. The regions are the continental U.S., the island of Oahu, the rest of the Hawaiian Islands, and the rest of the Pacific. Key Objectives: The community health workers are expected to operate in 4 geographic regions. The regions are the continental U.S., the island of Oahu, the rest of the Hawaiian Islands, and the rest of the Pacific. In these geographic locations, the contractor will provide at least 5 community health workers per region at 2.0 FTE (partial positions, work volume does not anticipate full-time positions). Over time the contractor and CPIVH will work to balance the positions according to workload. The contractors are expected to meet CPIVH-developed education requirements to promote VHA services. The contractor will provide a senior director with a background in Pacific Island cultural and health issues. The senior director will work with CPIVH staff in the development of the education materials for the community health workers. The contractors will utilize the CPIVH data platform to input results. The CPIVH staff will develop and maintain the data platform. Identify Pacific Island Veterans who are not utilizing VA services and help them to engage with and obtain access to healthcare services. Enhance access to VA healthcare services through community engagement and support. Solicitation is forthcoming. The solicitation will be available for download from https://sam.gov/content/opportunities. Interested vendors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the SAM website so that they are notified of the issuance of the solicitation and any amendments. The applicable North American Industry Classification System (NAICS) Code is 624190, Other Individual and Family Services, and the small business size standard is $16 Million. The procurement will be solicited as 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) set aside. Place of Performance: See Performance Work Statement
