Description
Sacramento County Department of Child, Family and Adult Services (DCFAS) practices in accordance with Assembly Bill (AB) 403 utilizing Child and Family Team (CFTM) meetings as a primary communication and engagement process between the child/youth and family, and the professionals responsible for assisting them. The Sacramento CFTM practice approach centers on utilizing two approaches: 1) Prevention CFTM and 2) On-going CFTM. Both focus on shared voices to assess, plan, intervene, monitor, and refine services with youth and families to either prevent entry into the Child Welfare system or if already involved with Child Protective Services (CPS), bring forth enhanced outcomes to support safe and timely reunification planning, lower level of care options starting with relative placements, pathways to alternate permanency options, and support planning for Transitional Age Youth (TAY). The County of Sacramento is soliciting proposals for a two fiscal year project from organizations that have a strong knowledge of Child Welfare, Continuum Care Reform (CCR), and Child and Family Teaming. Allocated funding for these services is in an amount up to $2 Million dollars ($1 Million per fiscal year). Organizations will develop and implement a CFTM program that provides the scheduling, coordination and facilitation of CFTM for families who come to the attention of Child Welfare in Sacramento County via the Hotline Reporting System (Prevention CFTM) and/or families with Child Welfare involvement (On-going CFTM). Services will be provided to youth/families who are not linked to specialty mental health care services and require utilization of the Safety Organized Practice (SOP) Consultation Framework
