Description
The Bridgeton Board of Education (district), Bridgeton, NJ is accepting proposals from qualified agencies to provide Physical Therapy. The services shall be well suited to meet the Individual Education Plan (IEP) service requirements of a K-12 school district’s special education students. B. SCOPE OF SERVICES The related service provider(s) for each service shall complete the Special Education Medicaid Initiative (SEMI) related paperwork in a timely manner as it pertains to district procedures. The agency shall be responsible for providing laptops and testing instruments when needed to complete their assignment. Speech and Language Therapy Services Speech and Language Therapy Services shall include, but are not limited to: Services performed in a direct, a consultative manner, or a combination of the two. Provide case management services for students that are determined eligible for special education due to a primary communication disorder. Attendance at the IEP team meetings for students served. Review data on progress towards goals Provide feedback to parents via an IEP progress report. Reports shall be provided as often as general education students receive report cards at a minimum. Become an active member of the Special Education Team at the schools where student services are scheduled. Review referrals for evaluation and eligibility Evaluate students to assist in special education eligibility. Complete procedural required paperwork within legally required timelines Submit paperwork to the District Education Office, parents and team members within the required timelines. The agency shall have English/Spanish bilingual speech and language therapists available.
